is looking for a Patient Care Coordinator to join our family.
Complete referrals for patients and coordinate with referring and receiving practices and/or departments.
May coordinate charity care with hospital.
Resolve patient billing issues with hospital.
Responsible for generating medical records for requests that come into the Clinic.
Develop and update processes for clinic referrals.
Educate patients on importance of keeping referral appointments.
Coordinate and schedule surgical procedures, diagnostic testing, or other services within various departments with ancillary providers.
Follow up on all referrals to ensure no care gaps/ Serve as medical assistant, lab or front office back up.
Participates in Good Samaritan Clinic outreach, fundraising and community development activities as a representative of Clinic programs.
Any additional duties as assigned.
